ABSTRACT

Objective:To perform cross-cultural adaption of the KING′s Parkinson′s Disease Pain Scale (KPPS), explore its reliability and validity in Chinese Parkinson′s disease (PD) patients, and to create the new version of the pain scale which adapts to the Chinese PD patients.Methods:This study enrolled 225 patients, including 121 men and 104 women who were selected from the Outpatient Center of Movement Disorders Clinic of the Second Affiliated Hospital of Soochow University from July 2018 to July 2020. All patients completed the evaluation of the Chinese Version of KPPS (KPPS-CV). According to the preliminary evaluation results, the expert group modified KPPS-CV after discussion, and developed a Modified KPPS-CV (MKPPS-CV). These patients then completed the MKPPS-CV evaluation during the 3-month follow-up. Cross-cultural adaptation was performed according to published international guidelines that include translation, back-translation, expert review, and pretesting. The following psychometric properties were evaluated: basic item analysis; floor and ceiling effects; construct validity; content validity; criterion validity (Spearman′s rho between the KPPS-CV and Numeric Rating Scale); internal consistency reliability (Cronbach′s alpha); test-retest reliability (intra-class correlation coefficient, ICC).Results:In item analysis, 50% of the items had poor discrimination (critical ratio<3.0), and floor effect was found in all domains (proportion of 0 point>15%). The items were reclassified after exploratory factor analysis. The content validity of item 3, item 10 and item 11 was low (item-level content validity index<0.78). Criterion validity showed the highest correlations (Spearman′s rho>0.88) between the KPPS-CV and Numeric Rating Scale. While overall scale reliability was minimally acceptable at 0.46, which showed a poor reliability of this scale. Test-retest reliability was excellent for each item (Spearman's rho>0.85). The Cronbach′s alpha of MKPPS-CV (0.76) was higher than that of KPPS-CV (0.46). It showed a great improvement after the modifying.Conclusions:When using scales that are not developed for local populations, differences in culture and clinical practices should be taken into account. MKPPS-CV is an acceptable, valid measure to evaluate pain in Chinese PD patients, which is more suitable for Chinese people.

ABSTRACT

Objective@#To investigate the incidence rate of transjugular intrahepatic portosystemic shunt (TIPS) complications in the treatment of cirrhotic portal hypertension, and analyze the cause of complication to management methods.@*Methods@#Data of 116 patients obtained from Zhongnan Hospital of Wuhan University were retrospectively analyzed. Portal venous pressure, routine blood test, coagulation test, liver and kidney function test, ammonia blood test, imaging and endoscopy reports were collected before and after procedure. The incidence rate of hepatic encephalopathy, gastrointestinal bleeding, ascites and shunt dysfunctions were observed. Data were expressed as mean ± Standard deviation and analyzed by t-test. A chi-squared test was used for comparison between categorical variables.@*Results@#The success rate of TIPS operation was 97.41% (113/116). Two patients underwent prompt TIPS procedure due to active bleeding. Bleeding was successfully stopped. Portal venous pressure of 113 patients decreased from (42.73 ± 7.64) cmH2O to (24.92 ± 7.60) cmH2O, and the difference was statistically significant (P < 0.01). Twenty cases were of hepatic encephalopathy. Preoperative level of Child-pugh class C patients was more susceptible to hepatic encephalopathy within 3 months after procedure than class A and B. After TIPS procedure, there were 22 cases of gastrointestinal bleeding, 18 cases of shunt dysfunctions and 26 cases of disease related death.@*Conclusion@#Rational patient selection strategies can effectively reduce portal venous pressure, incidence of hepatic encephalopathy, improve mid-and long-term therapeutic effects, and provide opportunities for liver transplantation.

ABSTRACT

Objective To explore the clinical value of transvaginal color Doppler ultrasound(TV-CDS)in the detection of blood flow changes within the ovarian stromal artery in patients with ovarian endometriosis.Methods Blood flow indices within the ovarian stromal artery were measured by TV-CDS in 60 patients and60 normal controls.Results In ovarian endometriosis group,TV-CDS examination showed the color signal pattern was dot-like with high-resistance ovarian stromal arterial flow which manifested significant higher resistance index(RI),pulsatility index(PI)and systolic/diastolic(S/D)ratio than those in normal group(P<0.01).Analysis on clinicopathologic data showed that cystic history and diameter were risk factors affecting the absence of ovarian stromal blood signal,while cystic history,diameter and category were associated with the significant difference of blood flow display area(P<0.05).Conclusions TV-CDS can be used as a non-invasive,convenient and sensitive method for assessing blood flow changes within the ovarian stromal artery,indicating ovarian interstitial damage as well as pathological conditions of ovarian endometriosis that contributes to clinical diagnosis and treatment.

ABSTRACT

<p><b>OBJECTIVE</b>Independently research and develop the wireless network system to monitor clinical nursing information in order to meet the needs of clinical care.</p><p><b>METHODS</b>The system consists of two sections which are the bedside machines and the central station. Advanced wireless network communications, microelectronics and computer technology are applied to continuously monitor various indicators of patients in intensive case units and general wards. Then the information was sent to the central monitoring station, which can automatically classify the data.</p><p><b>RESULTS</b>The system includes 4 subsystems which are automatic monitoring of urine output, automatic monitoring of infusion, automatic measurement of patient body weight and automatic management device of postoperative drainage tubes, which can provide essential indicators for major surgeries and critical patients.</p><p><b>CONCLUSION</b>The system has strong practicality. which can provide information for clinical care, ensuring the safety of medical care.</p>

ABSTRACT

Objective To optimize the extraction technique for Modified Danggui Buxue Concentrated Pills.Methods The extraction technique was studied by orthogonal design with the yield of astragaloside Ⅳ and total polysaccharides as the investigative indexes.Results The optimal extraction technique is as follows: adding 8-fold water,decocting 3 times and lasting 70 minutes for each time.Conclusion The optimized extraction technique is simple,reasonable and stable,which supplies experimental evidence for the further development of Modified Danggui Buxue Concentrated Pills.
